Purpose
To help individuals and communities better understand available healthcare resources and connect with appropriate healthcare providers or services when additional professional care is required.
What We Can Do
The programme can provide healthcare information support, map available community resources, facilitate appropriate referrals, assist with healthcare navigation and strengthen connections between communities and relevant healthcare organisations.
Internship & Experiential Learning
Interns can work on healthcare resource mapping, community research, field surveys, referral documentation, data management, community outreach and programme coordination.
Need of Support
The programme may require partnerships with healthcare providers, referral networks, trained volunteers, transportation assistance, information resources and community outreach support.
Expected Outcome
The programme aims to improve awareness of available healthcare services, make healthcare navigation easier, strengthen referral pathways and improve connections between underserved communities and appropriate healthcare resources.
